Services

Supports those affected by arthritis through public awareness, information and advice, direct support, promoting the issues affecting people with arthritis and promoting research.

A variety of specialised care services for people who are dying, includes nursing support, medical support and counselling services.

This service provides assessment, treatment and rehabilitation services for older people (aged 65+ & those close in age & characteristics).  Services are provided in hospital, outpatient clinics, and in the community.  

Disability Support Link (DSL)

Needs assessment and coordination (NASC) service for people with disabilities of any age:

  • Disability Support Services (DSS) -Needs Assessment and Service Coordination (NASC) service for persons from birth to 65 years. 
  • Health of Older People (HOP) 65+ – Health of Older People (HOP) is part of Disability Support Link (DSL) and provides needs assessment and service coordination for people with age-related disability lasting longer than six months who meet eligibility for publically funded services.   

The interRAI tool is used to assess a person’s needs and develop a plan of care in collaboration with the person and their family/whanau to meet their needs.

Services that are coordinated may include:

  • Household management
  • Personal care support
  • Day programmes (non-therapeutic)
  • Carer support/respite care
  • Residential care and support (change on level of care)
